Diamond At Home is a service offered as part of the resident’s discharge process. Diamond Pharmacy staff will deliver discharge prescriptions directly to the resident prior to leaving the nursing facility.
This is at no additional cost. Designed with continuity of care in mind, this program conveniently eliminates the need for getting prescriptions filled on the way home.
Additionally, when you fill your prescriptions with Diamond At Home, you can opt in to a post discharge follow up with a Diamond Senior Care Pharmacist to review your medication regimen, and answer any questions you may have.
